Thanks to the posts that advised me as to what forms I needed - I tried to download and although i have membership of TV , I could not download - in fact , I could not get the forms up on my screen - Looks like I will have to wait till I get to the Immigration Office to complete the forms

Thank you Rod but I am not a Surin resident - I live near Rayong (Klaeng) on the coast of the Bay of Thailand . And as I need to renew my annual visa , a visit is definitely required -
